Use this health coaching agreement template when working as a health coach to outline the scope of work, manage client expectations and get legal protection. Having this agreement in place makes both parties aware of their roles and responsibilities, and it helps to limit any misunderstandings or miscommunications to facilitate a better client-coach relationship.
This Health Coaching Contract (the “Agreement”) is signed by and between

The purpose of the Agreement is to develop a coaching relationship between the Client and the Health Coach in order to pursue the Client’s personal, professional, and/or business goals. It is also designed to create a strategy and/or plan to achieve those goals, with a view to maximizing the Client’s potential—both personal and professional.
Health coaching agreements are always for a specified period. Typically this period ranges from six to 12 months but can vary to meet this client’s specific needs.
The health coaching Agreement shall come into effect on the date the Parties sign it. It shall stand for a period of (write days, weeks, months, or years).
Including a renewal clause makes it easy for the client to continue with their sessions if they’re happy with the progress and results. Make sure to get the renewal in writing, signed by both parties, to ensure both parties agree to the terms of the extension.
Nothing in this Agreement shall be construed as requiring either of the parties hereto to extend or renew this Agreement or to enter into any subsequent agreements.
The parties may subsequently agree to extend the duration of this Agreement once the term of this Agreement has concluded. Any renewal or extension must be put in writing and signed by both parties.
It’s vital that both parties understand their responsibilities with regard to this agreement. This next section is a simplified scope of work (SoW) to clarify each party’s role within this agreement.

The Parties agree to meet for (number) sessions, with each of them lasting for (length).
The Client agrees to pay coaching fees to the Health Coach amounting to (number and currency) in exchange for coaching services.
When it comes to payment terms for health coaching services, you can use a few different payment structures. These include installment plans, payment per session, and even lump sum payments. Consider what works best for your business when specifying this in your contract. A fair payment period is usually between seven and ten days after receiving the invoice.
The Health Coach will send an invoice to the Client on the last business day of each month for the services provided.
Payment must be made within (number of days) days of receiving the invoice into the bank account provided on the invoice.
The Parties agree to keep all terms and conditions of the Agreement confidential unless their disclosure is required according to law. All conversations and information of this Agreement shall be protected, whereas the Health Coach may share personal information only with the Client’s permission.
Disclosing any information for purposes not defined by the Agreement is forbidden unless otherwise agreed upon by the Parties.
To ensure a healthy client-coach relationship, it’s important that both parties try to give fair warning when they need to cancel a session. Usually, between 12 – 24 hours is ideal and should allow the coach to fit in other sessions and offer the client a make-up session.
The Client agrees to inform the Health Coach (number) hours in advance of any session the Client needs to miss and/or cancel. In case of canceled sessions, the Health Coach reserves the right to charge the Client for a missed meeting. Depending on the Health Coach’s schedule, the Health Coach will try in good faith to find a new slot for the missed session.
Either the Client or the Health Coach reserves the right to terminate this Agreement at any time, ensuring to give a (number) days’ written notice. The Health Coach may request compensation for all services provided until the Agreement is terminated.
This section protects you as a coach in case something happens, such as injuries, adverse results, or other potential side effects.
The Health Coach shall not be liable for any damages, be it indirect, consequential, or special, suffered by the Client. Unless otherwise stated in the Agreement, the Health Coach makes no guarantees or warranties of any kind regarding coaching services the Parties agreed upon.
With respect to this Agreement, the Client agrees not to hold the Health Coach liable or responsible for any actions and/or inactions.
In the case that a court or a relevant jurisdiction finds any provision of this Agreement void and/or unenforceable, the remaining provisions shall still be valid in accordance with the intention of both Parties.
Each state in the US defines its own scope of work for health-related professionals. These cover the licensing requirements and credentials. Ensure you’re aware of the regulations in your state before entering into an agreement. Take special note of regulations pertaining to nutrition and fitness, as these areas show the most fluctuations between different states.
This Agreement shall be governed in accordance with the laws of (Health Coach.Country), without giving effect to its conflict of law principles.
This Agreement includes the entire agreement between the Client and the Health Coach. It also shows a complete understanding of the Parties with respect to this subject matter. After the Parties sign this Agreement, it will supersede all prior agreements, either written or oral, conditions and understandings.
The signed Agreement may not be amended or altered unless otherwise agreed upon by the Client and the Health Coach.
In the event a disagreement or dispute occurs in connection with this Agreement, the Health Coach and the Client agree to engage in the following:
